Important Update
Arthritis Foundation Self-Help Program
The Arthritis Foundation Heartland Region will no longer offer the Arthritis Foundation Self-Help Program. The Arthritis Foundation Self-Help Program originally developed by Dr. Kate Lorig has undergone several revisions in 2011 resulting in significant changes in how the Arthritis Foundation is able to offer the program. Beginning in 2011 the Arthritis Foundation Self-Help Program will be renamed to “Arthritis Self-Management Program” (ASMP) and will require a licensing fee for use of the program. At this time the Arthritis Foundation has two alternative options for this program: the Arthritis Self-Help Tool Kit or the Chronic Disease Self-Management program. The Arthritis Self-Help Tool Kit is available for purchase at our online store and is designed to assist individuals to manage their arthritis. The Chronic Disease Self-Management Program is very similar to the Arthritis Foundation Self-Help program. This program meets for 2 hours once a week for six weeks in a group setting; however, it includes all chronic health conditions. Quarterly Report Forms
The Arthritis Foundation is working in partnership with the Centers for Disease Control and Prevention to show how many people benefit from our programs. We rely on the quarterly attendance reports from our partners to measure program growth and to ensure continued federal support. Please use the Program Information Form and Participant Release Form each quarter for each class you offer.
