The Chronic Disease Self-Management Program
The Chronic Disease Self-Management Program (Living Well with Chronic Conditions) is a 2-1/2-hour workshop meeting once a week for six weeks. It is a community-based self management program that assists people with a chronic illness; such as arthritis, diabetes or heart disease in learning techniques to deal with health related issues and manage day to day activities to improve quality of life.
