Program Report Forms
In order to provide high quality programs, the Arthritis Foundation requires a small amount of paperwork from each site.
Site Agreement Forms
- Site Agreement forms ensure that both your facility and the Arthritis Foundation agree to the ongoing requirements for offering these programs in an appropriate facility with trained leaders. We are required to collect these agreements on an annual basis. After your first year as a program site, we will automatically move your new agreement due date to January for ease of tracking.
Participant Attendance & Report Forms and Participant Release Forms
- The Participant Attendance & Report Form must be completed for each class you offer. See the instructions for more information.
- The Participant Release Form must be completed for each new class participant.
- These forms should be submitted to Leslie Nelson at the Chapter Office at the end of each class series or for ongoing classes, no later than two weeks after the quarter ends (April 15, July 15, October 15 and January 15th).
Why collect this information? The Arthritis Foundation is working in partnership with the Centers for Disease Control and Prevention (CDC) to show how many people benefit from our programs. We rely on the quarterly attendance reports from our partner sites to measure program growth and to ensure continued federal support. In addition to providing important information to the CDC, statistics from active classes help us to find grant funding to underwrite the costs of program leader training, keeping costs well below industry averages.

Leader Certification
Leaders must complete a training workshop and teach six classes (about six hours) to become nationally certified. To maintain certification, leaders must teach at least six classes per year and complete a re-certification workshop every three years.
Already trained but need to complete your certification? After teaching your six class hours, fill out the certification application and send it in to receive your national certification.
