Welcome Arthritis Foundation Program Leaders!
On behalf of the 2.2 million people in our chapter area, we thank you for your work offering these valuable programs in your community. You make a difference to people with arthritis every day!
This page is dedicated to providing our current and newly trained program leaders with the resources to offer great Arthritis Foundation programs in your community! Many of the resources on this page are updated frequently, so we encourage you to bookmark this page and check back as you plan your health education topics or are looking for new information to share with your students. We also want to know if there are additional resources you would like to see on this page! If you have suggestions or requests, please let us know!

Program Paperwork:
- Leader Certification/Re-Certification
Once leaders have taught 6 classes (approximately 6 hours), they are certified for three years from the date of training. To maintain certification, leaders must teach at least one course series each year. If you've completed your six "student teaching" classes and are ready to certify, please complete the certification application form and send it on in! Check the staff contacts page if you are unsure where to send your completed form.
- Ongoing Program Paperwork
Thank you for your ongoing support of the Arthritis Foundation Programs by showing the success of your classes! Although paperwork may not seem like much of a contribution, the numbers you send in go a long way! By providing some basic paperwork about your class four times a year, you help us be accountable to our donors and to organizations who provide generous grant funding to offset the cost of supporting program trainings and maintenance.
Completing the required paperwork is a simple process to complete, and we are happy to help you with any concerns you may have about the forms you need to return to us. - Program Report Forms (Quarterly Reports) Quarterly reports are due on the 15th of January, April, July and October.
- Participant Waivers One waiver must be collected for each participant. Waivers are good for the lifetime of the participant while taking the class at your facility. Need ideas on how to collect these? Please don't hesitate to contact us!
