The Arthritis Foundation, Great West Region is here to support Programs for Better Living Instructors and Trainers in providing participants with quality programs. Please take a moment to get acquainted with some of the materials we have to help you make your program a success.
Quarterly Report Forms
The Arthritis Foundation works in partnership with the Centers for Disease Control and Prevention to show how many people benefit from our programs. We rely on the quarterly attendance reports from our partners to measure program growth and to ensure continued federal support. Please use the Participant Release Form (hard copy or electronic copy) and Program Information Form (hard copy or electronic copy) each quarter for each class you offer.

**Montana Program Instructors**
Program Instructors in Montana have a unique Quarterly Report form to complete. Please complete this Participant Release Form if you teach classes in Montana.
Marketing Materials
Customizable marketing materials are available to promote your classes. Brochures, ads, and flyers are available for each program. Contact your local staff person to request these materials.
